Physical Disability Support

Arthritis, physical disabilities, and age-related conditions are some of the few circumstances that prevent patients from caring for themselves. Our caregivers can assist you with daily tasks such as mobility, eating, exercising, and grooming.

Service area:

We provide care in the following Georgia counties: Appling, Bulloch, Candler, Emanuel, Evans, Jeff Davis, Johnson, Laurens, Liberty, Long, Montgomery, Tattnall, Treutlen, Toombs, and Wayne.

SOURCE and CCSP

The Community Care Services Program (CCSP) is a Georgia Medicaid home and community-based waiver services program that provides community-based social, health and support services to eligible consumers as an alternative to placement in a nursing home.
SOURCE is available to the frail elderly and disabled who are eligible or receiving SSI (Supplemental Security Income) or Georgia Medicaid. People under age 65 are eligible provided that they have a disability and are receiving SSI or Medicaid. If you or your loved one is receiving SSI or Medicaid you may be eligible for the SOURCE Program. To be eligible for the SOURCE Waiver, an individual must be 65 years of age and older, or if he or she is under the age of 65, must be physically disabled. As the waiver is intended to divert individuals from nursing home placement, a requirement is that they need nursing home level care. To make this determination, an assessment must be completed.

Eligible consumers may receive Georgia Medicaid-funded CCSP or SOURCE. Community Home Nursing Services is a provider of these two services:
Personal Support Services - In-home services such as personal care, meal preparation, light housekeeping, shopping and in-home respite services.
Structured Family Caregiver is based upon eligibility and provides support to family caregivers who live with and assist an elderly and/or disabled waiver participant requiring significant assistance with activities of daily living.
What is a Medicaid Waiver? For persons with limited financial resources, Medicaid pays for nursing home care. For those who wish to live at home or in assisted living, sometimes Medicaid will pay for care in those locations if it can be obtained at a lower cost than in a nursing home. It does this through "Medicaid Waivers."

CCSP and SOURCE are Medicaid waiver programs that provide a range of community-based services to assist individuals who are older and/or have disabilities to achieve safe, self-reliant lives and allow them to live at home instead of a skilled nursing facility. Community Home Nursing Services is a Georgia-approved, participating provider of at-home care services to adults eligible to receive the CCSP and SOURCE waiver. This means that persons who qualify for the CCSP program may receive home care or skilled nursing care services at no charge or a small monthly cost-share.

Skilled Nursing Care
Respite Care
Errand Assistance - i.e. shopping for essentials
Assistance with meal preparation
Assistance with day-to-day activities
Bathing
Personal Care

Or you may contact To apply for this program, individuals should contact their local Area Agency on Aging. One can also call 866-552-4464. Once the AAA has screened the applicant and there are available slots for new participants, the applicant must complete the Medicaid application through the Division of Family and Children Services, if they are not already enrolled in Georgia Medicaid.
